As a student requesting learning accommodations through the Academic Support Center, I understand that I have responsibilities to both myself and my education here at the Florida Institute of Technology. I understand that learning accommodations are put in place to provide me with the necessary tools and strategies that I may use to work towards my academic success at Florida Tech. As part of my request to receive accommodations, I agree to the following:

1. Meet with my academic advisor to develop an academic plan to follow my degree at Florida Tech and keep this academic plan updated each semester as I complete the necessary coursework as it pertains to my degree.

2. Complete necessary paperwork in accordance with the Family Educational Rights and Privacy Act (FERPA).

3. Follow my academic plan or consult with my academic advisor before adding/dropping course(s).

4. Attend classes regularly, regardless of whether or not a formal attendance policy is established for a class.

5. Complete assignments in a timely fashion for all courses for which I am registered.

6. Arrange any quizzes or exams in a timely and responsible fashion through the Academic Support Center if my accommodations require use of the ASC as an alternative testing site

7. Meet with my academic advisor to discuss my schedule and semester expectations as well as midterm grades and possible progress reports submitted by my instructors.

8. Contact the Academic Support Center if I encounter personal, academic, or financial difficulties, which may interfere with the successful completion of my course-of-study.

9. Maintain communication with my professors and advisors through my fit.edu email account in accordance with expectations published in the Florida Tech Student Handbook.

10. Maintain any necessary appointments through CAPS if indicated through requested accommodations.

Students who neglect one or more of the above will be required to speak with the Director of Academic Support and reassess their commitment to their education at Florida Tech as well as the need for specific learning accommodations. Students who neglect their academic responsibilities as noted above and who refuse to meet with ASC staff will be assumed to have waived their need for accommodations for the present academic term.
