Student of Concern Reporting
The goal of Student of Concern Program at CAPS is to identify students in the Florida Tech community who appear to be having difficulty adjusting to campus life and intervene before the student reaches a crisis level. In addition to contacting CAPS by phone or visiting CAPS to meet with the CAPS Director and/or Assistant Director, an anonymous form can be filled out below.
All Concerned Student Reports will be received anonymously by the CAPS Director and Assistant Director. Once received, we will evaluate the information and determine the most appropriate way to work with the individual student. If you know of a student who you are concerned about, please fill out the form below including the name of the student and a behavioral description that causes you to be concerned. Additional identification data such as room number, full name, student ID, major, may be useful, although not necessary. Feel free to add any other relevant information that may be helpful for us in assisting this student. This system is anonymous unless you would like us to contact you.
