Transcript Request Form This transcript request form is for the ABA Online Program with the Continuing Education Department
Transcripts can be requested online or by faxing or mailing a written form. The cost for each transcript is $5 and can be paid by credit card or check. Expedited shipping is available for an additional fee.
To send your transcript to the BACB enter the following in the issued to field in either form:
You can request a transcript either by form or in the online shopping chart. This option is for returning students who have set up a registration username and password. The password for the new registration system is not your Florida Tech TRACKS password by default. If you are a returning student and have not already set up your registration password and security question, please call 1-321-674-8382.
