Master's Degree Program Plan
Each master's-level graduate student is required to have an approved program plan on file in their department no later than one month prior to the time that nine credit hours of graduate courses have been completed. This form must be submitted to the Office of the Registrar along with the Petition to Graduate form.
Petition to Graduate
Required form to initiate graduation and must be accompanied by degree program plan signed by your academic unit. Complete this form and return to Office of the Registrar.
Publishing your Dissertation The microfilm publishing agreement form is required for all Doctoral students. You may pick one up in the Office of Graduate Programs. Please complete this form and turn in to the Office of Graduate Programs when turning in the final copies of your dissertation.
