Domestic students who are registered for 6 or more credits qualify to participate in the University’s student health insurance, and can enroll during the open enrollment period by completing the Student Health Insurance Enrollment/Waiver Request Form and submitting it to the Campus Services office by the appropriate deadline.
Domestic students who wish to enroll after the open enrollment period must provide documentation of involuntary termination of previous health insurance coverage within 30 days of their insurance termination, complete the Student Health Insurance Enrollment/Waiver Request Form and submit documents to the Campus Services Office.
Domestic students who have
their own comparable health insurance coverage may waive University Health Insurance charges by completing the waiver portion of the Student
Health Insurance Enrollment and Waiver Form and
providing proof of coverage (photocopy of both the front
and back of their current insurance identification card
or a copy of the endorsement page of their current plan).
This form, together with proof of insurance, must
be submitted to the Campus Services Office no later that 5 PM, Friday, August 31, 2007
for Fall Semester 2007.
For students who do not attend
Fall Semester 2007 classes, but will be starting
school Spring Semester 2008, the completed form and proof
of insurance must be submitted no later than 5 PM, Friday,
January 18, 2008.
For students who do not attend
Fall Semester 2007 or Spring Semester 2008 classes, but
will be starting school Summer Term 2008, the waiver
and proof of insurance must be submitted no later than 5
PM, Friday, May 23, 2008.
Proof of Insurance (copy of insurance card front and back) may be submitted to Campus Services by mail, fax or hand delivered.
In all cases, students
who fail to submit the required documentation by the dates
indicated will be automatically billed and enrolled for
student health insurance and are obligated for the entire
Academic Year or any portion thereof remaining at time of
registration.
There is no socialized or
national system of health care in the United States, and
medical treatment is expensive. All Florida Tech students
are required to have appropriate medical insurance coverage.
As part of your tuition and fees, you will be charged for
student health insurance. This means you will automatically
be covered though Florida Tech's student health insurance
plan unless you are officially sponsored by your home government
or agency that guarantees your health insurance fees as
part of their contract with the university. Students in
this category will not be charged for the university's coverage.
It is mandatory for all other
international students to be covered by the university's
health insurance plan. Exceptions may be granted only if
you have an insurance plan that meets very strict requirements
in order for you to qualify for the waiver. You may request
a waiver of this fee by completing a form and providing proof
of appropriate insurance to the Office of International
Student and Scholar Services.
Florida Tech will not
accept coverage by an insurance company
outside the United States. Before enrolling
for coverage in an insurance plan other
than Florida Tech's student health plan,
please check with the Office of International
Student and Scholar Services to determine
if it meets the waiver requirements.
