Student Health Insurance
Important Notice!
Student Health Insurance coverage is MANDATORY for all full-time undergraduate (12 credits or more) and graduate students (9 credits or more), and will AUTOMATICALLY be charged to the student's account unless properly waived. U.S. government guidelines require health insurance coverage for both full-time and part-time International students, including coverage for all dependants residing with them in the United States. University Alliance enrolled students are not eligible to participate in the Florida Institute of Technology Student Health Insurance program.
Students Seeking to Enroll
For the 2011-2012 Academic Year, the University's Student Health Insurance Coverage begins August 10, 2011 and ends August 09, 2012. Domestic students who are registered for 6 or more credits may choose to purchase student health insurance by checking the ENROLL option on the Student Health Insurance Enrollment and Waiver Form (Campus Services Office). The form must be completed by the appropriate open enrollment cut off date for each semester.
Students seeking to enroll after the open enrollment period must provide documentation of involuntary termination of previous health insurance coverage within 30 days of insurance termination.

All procedures require a claim form to be considered for reimbursement. Please complete and submit https://www.bollingercolleges.com/ClaimForm/default.aspx for any health care costs incurred.

Student Health Insurance Cards
If you sign up for the Student Health Insurance, your cards will be mailed to the address given on your application. Please check with Campus Services under the Evans Dining Hall next to the Ratskeller if you have not received your card by October 1, 2011 or call 321-674-8076.
