Employee Benefits
Florida Tech faculty and staff earn both pay and benefits for their work at the University. These two elements together make up your total compensation package. We enjoy a very rich insurance package supplemented by Florida Tech, as well as a number of optional insurances from which to choose.
Please note: If you elect Health Insurance coverage, you are eligible to enroll immediately upon hire. Your coverage ends at midnight on the last day of your employment.

Eligibility for Benefits
You are eligible for coverage under our Health Insurance plan immediately upon hire. You are eligible for coverage under all other insurances effective the first day of the calendar month following or coinciding with 30 days of continuous active service for full-time employees authorized to work at least 30 hours per week. You will become insured on the date you elect any of the benefits by completing a required enrollment form and by signing an approved payroll deduction form, but no earlier than the date you become eligible. If you do not enroll within 31 days after a change in status as determined by the employer, you will not be allowed to enroll until the next annual enrollment period. If you are not in active service on the date you would otherwise become insured, you will become insured on the date you return to active service.
All employees, with the exception of students, are eligible for participation in the Florida Institute of Technology 403(b) Plan, after completion of thirty (30) days service.
Changes in Coverage of Pre-Tax Benefits During the Year:
The benefit choices you make can be tailored to your personal needs. If you enroll in benefits on a pre-tax basis, you may not add, delete, or change the coverage you have selected for yourself or your dependents until the next open enrollment period. The only exception is if you experience a qualifying family status change. A qualifying family status change includes:
- Marriage or divorce
- Birth or adoption of a dependent child or a change in custody
- Death of a spouse or a dependent child
- Your spouse has a change of employment or status which affects benefits coverage
- You change your employment status which affects your benefits status
- You experience an involuntary loss of other group benefits coverage
You must report a family status change in writing to the Benefits section of Human Resources within 30 days of the qualifying event.

Dental Insurance
Florida Tech provides employees and eligible dependents the opportunity to elect dental coverage through Humana Dental Plans. There are three plan options: DHMO, PPO Low with in and out-of-network coverage, and PPO High Plan. If you choose the DHMO Plan you must select a primary dentist for each covered person.
Eligible dependents include:
- A legal spouse
- Unmarried, dependent children for whom the employee provides principal support. Eligibility for coverage extends to age 25 if child is an IRS dependent AND living in the employee household OR a student. No child can remain on the plan beyond December 31st of the year in which they turn 25.
- Uunmarried, dependent child incapable of self-sustaining employment by reason of mental retardation or physical handicap.
- Certified domestic partners.
- Children of certified domestic partners subject to the same eligibility standards applicable to dependents.

Employee Assistance Program (EAP)
Florida Tech offers Employee Assistance program through Magellan Health Services, a partner with Blue Cross/Blue Shield. Services include EAP Counseling,Caring for children or aging parents, Dealing with conflict or violence, Mind and body discount programs and online tools and information.
- All benefit eligible employees and members of their household can participate.You do not need to be enrolled in health insurance to use the services of EAP.

Health Insurance
The benefits and coverage described herein are provided through a trust fund established and funded by a group of employers. It is not an insurance company and is not protected by a guaranty fund in the event of insolvency. Participating employers are assessable for any losses incurred by the trust. Benefits are provided by ICUBA, Independent Colleges & Universities Benefit Association, and administered by Blue Cross/Blue Shield.
Please note: If you elect Health Insurance coverage, you are eligible immediately upon hire and it ends at midnight on the last day of your employment.

Prescription Drug Benefits Walgreen Health Initiatives
Walgreens Health Initiative (WHI) administers the pharmacy benefit for all of the medical plans. The pharmacy benefit includes value based generics, medication therapy management and clinical management, and separate website and customer care line from the BCBS medical. WHI has the same parent company as Walgreens retail pharmacies. However, members can use any of WHI's 62,000+ network pharmacies nationwide. A separate identification card is issued for pharmacy benefits. There are three ways to fill a prescription (each with a different copay schedule): 30 days supply at the retail pharmacy; 90 days supply at the retail pharmacy; or 90 days supply through mail order.

Mail Order Drug Program (Walgreens Health Initiatives) - If you are taking a maintenance prescription for conditions such as arthritis, high blood pressure, or diabetes, among others, you can benefit by using the Walgreens Health program. Savings Bonds Interest Rates
New rates will be posted November 1st every year. The Treasury Department announced that Series EE Savings Bonds issued on and after May 1, 2005, will earn fixed rates of interest. The new fixed rate will apply for the 30-year life of each bond, which includes a 10-year extended maturity period; unless a different rate or rate structure is announced and applied at the start of the extension period.
