Motor Vehicle Request Form:

Anyone operating university owned motorized property must submit the completed form and be approved prior to driving. This form must be completed and returned with a legible copy of your driver's licence and Florida Tech ID card to Claire Wurmfeld or Jennifer Ledford r delivered by campus mail. Once we recieve the results from the DMV, you will be notified of your eligibility to drive univeristy vehicles. Please allow 10 days for processing the request. Download

Vendor Insurance Requirements

University Owned or Rental Car Claims Reporting Procedures

Any claim involving university or rental property must be reported to Financial Affairs within 24 hours of the incident whether on or off campus. Resulting faculty, staff or student injuries must be reported to financial affairs in the same manner. Financial affairs is located in the John Miller building #401. Workers Compensation - Timely reporting of employee injuries is crucial. All incidents must be reported to the Insurance Specialist in a timely manner. Once the incident is reported the Insurance Specialist will contact you.

Special Event Insurance - (TULIP)

If you are having an event on campus you must contact the Director of Conferences and Events, Erica Spencer at spencere@fit.edu

All third parties who are participating in an event should have a contract for services. This is critical to protect our institution and to make sure that all parties have the same understanding with respect to the venue, who is responsible for what, what the event is, when the event is, and so on.

Liability Coverage is administered by Bene-Marc and Entertainment Brokers International (License #0773887). The pre-rated TULIP program is underwritten by Clarendon Insurance Company. Other coverage may be written by other underwriters.

The TULIP is a Tenant's and User's Liability Insurance Policy, also known as "Events Coverage". It is used by institutions who permit "third parties" to use their facilities for specific events. It is NOT intended for institutions to shift any of their specific operational risks away from their own General Liability Policies.

It protects both the Facility user and the Institution against claims by third parties who may be injured or lose property as a result of participating in an event.

Events may range from very low risk events such as classroom seminars, receptions or weddings to higher risk events including camps, sports events or rock concerts.
