Attendance
Confirmation Form
This form reserves your space in the incoming class. An Attendance Confirmation Form must be completed and accompany a $300
nonrefundable tuition deposit. This deposit will be applied to your first semester tuition. You can print this form and mail it with a
check or use a credit card and submit the form and tuition deposit online (see below).

Flight Student
Checklist
New students admitted for a flight prgoram must complete and submit this checklist along with the Attendance Confirmation form and
tuition deposit to the Office of Admission by the May 1 common decisions deadline, or within 30 days of acceptance (for letters dated
after April 1).
Freshman Housing & Meal Plan Contract
All new students seeking on campus housing and meal plans must submit these forms with their $200 housing deposit. Download, print and
mail these forms with the $200 housing deposit. All freshman and transfer students with less than 24 earned semester hours are required
to live on campus. For exemption from this requirement, please refer to the Freshman Residency Requirement.

Student Health Insurance Information
Get information about or request a waiver for university health insurance. All students attending Florida Tech must have health
insurance coverage. Florida Tech offers health insurance for students who do not have their own coverage.

Submit the Student Health History Form prior to arrival on campus.
FAFSA Form
Apply For Financial Aid! To be considered for all types of financial aid, you must complete the FAFSA. You can get a copy from any high
school guidance office, any college financial aid office, or you can complete the online version. Our Title IV code for the FAFSA is 001469.
I-20 Form
(International Students Only) Upon admission, and with appropriate financial documents, Florida Tech will issue an I-20, that is, a
"Certficate of Eligiblity for Non-Immigrant Status". This certificate is the document you will present to the U.S. Consulate officer in
your country to obtain an F-1 student visa. If you did not receive your I-20 with your admission packet, check the memorandum that
comes with your letter of admission to see which documents you are missing. Also you can access the "I-20 Requirements" form and send the rquired documentation.
International Transfer Eligibilty Form
All International transfer students who are attending another school in
the U.S. must complete this form if requesting an I-20.
