Additional Forms for Applicants

Here are some additional forms that applicants will find useful when applying to Florida Tech.

  • Transcript Request Form
    All Freshmen and Transfer applicants must complete this form and present it to the guidance office at your high school or the registrar at your college/university. Have your transcripts sent directly to the Florida Tech Office of Undergraduate Admission.
  • Letter Of Recommendation
    Letter of Recommendation Form for all Undergraduate Admission applicants. Send this form to your recommender to complete. Two letters of recommendation are recommended.
  • Alumni Grant Form
    Have a graduate of Florida Tech submit this form on your behalf and you can earn a $1000 grant renewable annually for up to four years. This form must be received by January 15th, prior to your fall enrollment. This grant applies toward tuition for full-time undergraduate study on the Melbourne campus of Florida Tech.
  • I-20 Request Form (International Students)
    All International applicants must complete and submit this form if requesting an I-20.
  • Request For Financial Support (International Students)
    International students who apply before January 15 who have not taken or cannot take an SAT I or ACT, do not meet the minimum criteria for the Florida Tech Scholarship program, or need additional financial support, may be eligible for a Florida Tech Grant if you complete this form
  • Transfer Eligibility Statement (International Students)
    All International transfer students who are attending another school in the U.S. must complete and submit this form in order to receive their I-20.
  • Essay Questions
    Students must complete a one page essay in response to one of three questions, available here.
