Name, ID, and Address Changes

Name, ID, and Address Changes

A Florida Tech Change of Name Form must be completed before making Name or ID changes .
Name or ID changes are not permitted without legal documentation.
A driver’s license or Social Security card is not considered sufficient documentation.
Approved documentation for name changes include: certified copy of marriage license, court order, or dissolution decree reflecting the new name in full.
For non-US citizens, current passport or official proof of identity, certified by US embassy abroad or by the appropriate foreign embassy in the US.

Changes in Banner can only be made by the appropriate office listed below:

Employees: Human Resources makes the change
Student Employee: Registrar or Human Resources makes the change (i.e., college work study, college roll, GSA)
Employment Applicant and not a Student: Human Resources makes the change
Student and not an Employee: Registrar's Office makes the change
Student Applicant; not a Student or Employee: Admissions makes the change
Financial Aid Applicant and not any of the above: Financial Aid makes the change
Vendor and not an Employee and not a Student: Purchasing or Accounts Payable makes the change
Alumni : Alumni requests the change in writing to the Registrar's Office

Department recommendation:
It is recommended that the following departments run Banner report GWR100R to access Name and ID changes made during a period of time.
The report will show: NAME, ID, PIDM, TYPE, USERID, APPLICATION, and ACTIVITY DATE.
It is also recommended to periodically cross-reference this report with department records to verify integrity and make appropriate changes or notations to paper documents.
