How to set up a Strive account:

Employee Instructions

  1. Log into your BCBS My Health Toolkit
  2. Click on the wellness tab on the top of the webpage
  3. From the drop down select Strive
  4. Click the blue log-in link
  5. Follow the instructions to make your account

Employee Spouse Instructions

  1. Log into the MYHealth Tool Kit Portal
  2. Click Register Now
  3. Use your insurance card to start the registration process
  4. You will need your insurance card number and your date of birth
  5. Complete the registration process
  6. Click on the wellness tab on the top of the webpage
  7. From the drop down select Strive
  8. Click the blue log-in link
  9. Follow the instructions to make your account

Dental Insurance

Florida Tech provides employees and eligible dependents the opportunity to elect dental coverage through Delta Dental Plans. There are two plan options: Base Plan and Buy Up Plan.

Additional Life Insurance

You may enroll in, or increase your additional life insurance up to five times your annual salary not to exceed $500,000. Evidence of insurability is required.

  • Basic and Voluntary Group Life and AD&D Insurance
  • Please Note:  Any election or changed coverage amount to Voluntary Employee or Spouse Life Insurance does require Evidence of Insurability (EOI) and must be approved by Symetra. Click on the link here to complete the EOI. The link will also be on the review page at the end of your enrollment process in Workday - Evidence of Insurability Website

  • Is your spouse or dependent child also an employee of Florida Tech covered under their own employee life insurance plan?  Please note you may not elect coverage for Your Dependent if such Dependent is covered as an employee under The Policy. Also, when insuring dependent children please note no person can be insured as a Dependent of more than one employee under The Policy.  For more information please see the Basic and Voluntary Group Life and AD&D Insurance link above.

Pet Insurance

Nationwide Pet Insurance offers a discounted rate to ICUBA schools.  This is a discount program that does not run through payroll.  Nationwide will directly bill you.  Nationwide Pet Insurance will allow you to access ANY VET.

View more information on Pet Insurance to obtain information regarding the plan designs and rates to request additional information directly from Nationwide.  Premiums vary based on the age of the pet, species, size, plan type and state of residence.  Some conditions may not be covered and underwriting is required for each Pet enrolled in the Plan.

Short-Term Disability

Short Term Disability coverage is provided at no cost to benefits-eligible employees. Under Short Term Disability, a disabled employee will receive 66 2/3% of salary up to a maximum benefit of $2,000 per week for a maximum of 11 weeks.

Long-Term Disability

Core Long Term Disability coverage is provided at no cost to benefits-eligible employees.  This plan provides a benefit of  60% salary up to a maximum of $2,000 per month.

Long Term Disability Buy–Up coverage is available and is purchased by the employee through payroll deduction. This plan provides a benefit of   67% salary up to a maximum of $14,000 per month.
