Florida Tech offers comprehensive medical benefits provided by Blue Cross Blue Shield of Florida through customized plan designs by ICUBA. Here are the 2020 - 2021 plan options: Preferred PPO Blue Options, Premier Copay Blue Options, and $4000/$8000 Deductible Plan

How to set up a Rally account:
Employee Instructions

  1. Log into your CUBA Benefits portal
  2. Under ICUBA Single Sign-On click on BCBS MyHealth Tool Kit Portal
  3. Click on the wellness tab on the top of the webpage
  4. From the drop down select Rally
  5. Click the blue log-in link
  6. Follow the instructions to make your account

Employee Spouse Instructions

  1. Log into the MYHealth Tool Kit Portal
  2. Click Register Now
  3. Use your insurance card to start the registration process
  4. You will need your insurance card number and your date of birth
  5. Complete the registration process
  6. Click on the wellness tab on the top of the webpage
  7. From the drop down select Rally
  8. Click the blue log-in link
  9. Follow the instructions to make your account

Optum Rx:

  • Prescribed diabetic supplies including meters, lancing devices, lancets, test strips, control solution, needles, and syringes
  • Aspirin for adults with a physical prescription
  • Prescribed generic folic acid and generic pre-natal vitamins for pregnancy

Optum Rx administers the pharmacy benefit for all of the medical plans.

The Optum Rx Prescription Medication Plan's benefits include:

Our ICUBAcares Pharmacist Advocate Program (PAP) is comprised of Real Pharmacists who are Real Advocates with Real Solutions! Our ICUBAcares PAP will assist members with closing gaps in care attributed to Prior Authorizations, Quantity Limits, Step Therapy and Generic Drug Requirements; assist members with formulary changes; serve as a liaison between doctors, pharmacy and insurance company; encourage members to use therapeutically appropriate, cost-effective therapy; and more.

Dental Insurance

Florida Tech provides employees and eligible dependents the opportunity to elect dental coverage through Humana Dental Plans. There are three plan options: DHMO, PPO Low with in and out-of-network coverage, and PPO High Plan. If you choose the DHMO Plan you must select a primary dentist for each covered person.

 Vision Insurance

Florida Tech provides employees and eligible dependents the opportunity to elect vision coverage through EyeMed. The EyeMed Vision Plan offers a benefit option to cover routine eye care, including exams and eyeglasses or contacts.

Additional Life Insurance

This year you may enroll in, or increase your additional life insurance up to five times your annual salary not to exceed $500,000. If the value of your coverage is $150,000 or less, evidence of insurability is not required.

Pet Insurance

Florida Institute of Technology now offers Pet Insurance! This is a post-tax benefit that can be deducted from your payroll check.  Veterinary Pet Insurance will allow you to access ANY VET and include protection for :

  • Hereditary conditions
  • Procedures/services
  • X-rays

Short-Term Disability

Short Term Disability coverage is provided at no cost to benefits-eligible employees. Under Short Term Disability, a disabled employee will receive 66 2/3% of salary up to a maximum benefit of $2,000 per week for a maximum of 11 weeks.

Long-Term Disability

Core Long Term Disability coverage is provided at no cost to benefits-eligible employees.  This plan provides a benefit of  60% salary up to a maximum of $2,000 per month.

Long Term Disability Buy–Up coverage is available and is purchased by the employee through payroll deduction. This plan provides a benefit of   67% salary up to a maximum of $14,000 per month.
